Emergency Pet Caregiver Agreement

This Emergency Pet Caregiver Agreement records the arrangement between a MyPetAlert member (the “Owner”) and the person the Owner designates to care for their pet(s) in an emergency (the “Caregiver”). It supports the Owner’s MyPetAlert emergency plan: if the Owner cannot get home and their MyPetAlert identifier is used, MyPetAlert helps alert the Caregiver named below.

The Caregiver will care for the animal(s) beginning on the date the Caregiver is contacted in an emergency, and until the Owner is able to resume care or otherwise notifies the Caregiver.

If the Caregiver Cannot Help

If the Caregiver becomes unable to care for the pet(s), the Caregiver will contact ____________________ (name) at ____________________ (phone) to arrange substitute care and will promptly notify the Owner. If arrangements cannot be made, the Caregiver will contact the Owner’s alternate caregiver listed in the Owner’s MyPetAlert account.

Consent and Agreement

The Caregiver confirms they have agreed to serve as the Owner’s emergency pet caregiver and consent to be contacted by MyPetAlert (including by phone and text) for that purpose. This agreement is the entire agreement between Owner and Caregiver; any change must be in writing.
